Solvista Health Member Advisory Council (MAC) Application
Solvista Health is looking for current and former clients, family members, and other interested people to share their lived experience and serve on our Member Advisory Council (MAC). This committee meets once a month to discuss member’s experiences with Solvista Health and to share ideas to help create the best possible environment of care. We need members in Chaffee, Custer, Fremont, and Lake Counties. If you are interested, please complete and submit this online form. A Solvista Health staff member will respond to your application. You can also pick up an application form from the front desk of any location or ask a member of your clinical team and complete the application at your convenience. Questions?
